Role of Endothelial Shear Stress in the Natural History of Coronary Atherosclerosis and Remodeling

Author: Yiannis S. Chatzizisis, MD, Harvard-MIT Division of Health Sciences and Technology, Massachusetts Institute of Technology, Cambridge, MA, USA

CONCLUSION

  • Low ESS predicts the development of TCFAs through enhancement of lipid accumulation, inflammation and subsequent upregulation of matrix degrading proteases
  • Excessive expansive remodeling self perpetuates a vicious cycle of low ESS and plaque inflammation, thereby facilitating the differentiation of early plaques to TCFAs
  • Low ESS may be utilized in man for identification of TCFAs at early stages of development before they rupture. Early identification of TCFAs may facilitate the pre-emptive local intervention to avert future acute coronary events
