Feasibility and results of novel side branch evaluation by customizable measurement planes in OCT – A SORT-OUT V substudy

Author: Niels R. Holm, MD, Aarhus University Hospital, Denmark

BACKGROUND

  • SB intervention after jailing is guided by
    • TIMI flow
    • Ostial DS% (eyeball) – FFR
  • Some jailed SBs gain ostial diameter at 8m FU
  • SB FFR gain also detected

AIM

  • Verify if jailed SBs increases in ostial area
  • Assess feasibility of OCT and the Cut Plane technology (QAngioOCT Research Edition 1.0, Medis Specials bv, Leiden, Netherlands) for larger scale SB assessment by main vessel OCT acquisitions

CONCLUSION – SB OSTIA

  • Ostial SB MLA was insignificantly increased at 12m FU after jailing by DES
  • Cypher tended to have a larger increase in ostial SB MLA compared to Nobori DES
  • Neointimal bridging was a common. Clinical implication unsettled

CONCLUSION – FEASIBILITY

  • SB ostial assessment from MV OCT was feasible
  • Feasibility was limited by:
    • Wirer shadows
    • Tissue shadowing properties
    • Insufficiently cleared blood in lumen
