STENT LONGITUDINAL DISTORSION DURING LEFT MAIN STENTING

Aorto-ostial stent crush during LMCA stenting Exploratory bench findings

Author: Niels Ramsing Holm, Aarhus University Hospital, Denmark

SUMMARY

  • The guiding catheter is constrained by the jailed wire and is tracked towards the stent edge
  • Regular movements of the guiding catheter during PCI could cause bending and longitudinal compression of the stent. Stent platforms may differ in resistance to distortion
  • Early wire exchange after POT may reduce this risk
  • Accidental abluminal rewiring during wire exchange may lead to ostial stent crush
  • Final Ostial Optimisation Technique may correct guiding catheter induced ostial bending and malapposition of the LMCA stent
