TAKE HOME MESSAGES

  • The presence of a bifurcation or a trifurcation at the proximal or distal entry point of the CTO has been described as an anatomical predictor of failure.
  • The incidence is 25%.
  • Different techniques available to overcome this problem.
  • Wiring of the side branch at baseline condition and the absence of dissection affecting the bifurcation were powerful predictors of technical success.
  • The rate of technical success (main vessel and side branch) is 74%.
  • A two-stent technique may be consider up-front for bifurcation for large side branch.
  • Choice of strategy depends on lesion and vessel characteristics (plaque distribution, the diameter of the branches, angle between them, and anatomy of side branch).
  • TAP is a relatively easy double stent technique. However the angle needs to be suitable
