CONCLUSION
- Both Culotte and DK-Crush require complicated processes in order to obtain optimal results.
- I propose reconsideration on stent size selection in the LM bifurcation.
- We should consider the usefulness of the distal optimization technique (DOT) to be added later as well as proposing a method of low pressure placement of a large size stent.
- The reason why the DK – Crush is giving better clinical results seems to be due to the fact that it is possible not to reduce the incomplete stent apposition volume at the bifurcation but to obtain a larger vessel diameter in the treatment area by very high pressure expansion.
- In the DK – Crush, it is necessary to reduce the area of three overlapping struts as much as possible, and therefore it is necessary to consider the proximal end design of the side branch stent.
