European Bifurcation Club 2019, EBC 2019 - Barcelona, Spain LM SESSION Assessing Optimal Percutaneous Coronary Intervention for Left Main Coronary Artery Stenting Registry Author: Kazushige Kadota, MD, PhD, Kurashiki Central Hospital, Japan SUMMARY 1 The Long-term outcomes ager stent implantation for unprotected LMCA lesions were not determined by the bifurcation lesion types, but were related to the 2- stent strategy. Also, the difference in the rates of TLR and ST between 1-stent and 2-stent strategies were observed within 1 year. SUMMARY 2 Drug-eluting stent implantation ager rotational atherectomy was a safe and feasible strategy for complex bifurcation lesions. In this strategy, the 2-stent approach was associated with markedly worse 5-year clinical outcomes than the 1-stent approach. The events rate within 1 year were significantly higher for all outcomes in the 2-stent strategy group than in the 1-stent strategy group. CONCLUSION The AOI-LMCA registry confirmed that a simple stenting strategy could be recommended for treating leg main bifurcation lesions in real world practice. Meticulous care should be paid within 1 year after using a complex stenting strategy.
European Bifurcation Club 2019, EBC 2019 - Barcelona, Spain HOW TO TREAT (by EBC experts) - VOTING How to treat an Ostial LAD Bifurcation Lesion Author: Vladimir Dzavik, MD, PhD, University of Toronto, Canada 46 year old man Recent diagnosis of diabetes or pre-diabetes Started on metformin Several month history of exertional dyspnea and chest discomfort Attributed to being unfit Presented to a local hospital with anterior chest pain and dyspnea at rest
European Bifurcation Club 2019, EBC 2019 - Barcelona, Spain TIPS AND TRICKS IN BIFURCATION PCI - THE SYNERGY BETWEEN EBC AND ASIA Use of DES and DCB in LM Bifuration Lesion Author: Zhanying Han, MD, First Affiliated Hospital of Zhengzhou University, China BACKGROUND Although several stenting techniques are available for the treatment of de novo coronary bifurcation lesions. The treatment of coronary artery bifurcation still represents a challenge for interventional cardiologist. over the past years, different studies have proposed and analysed the use of drug-coated balloon (DCB) in the treatment of coronary artery bifurcation lesions as an alternative treatment. CLINICAL CHARACTERISTICS Zhang Q Female 76years Paroxysmal chest tightness for 10 years Agravated for 20 days Risk factor: Diabetes 1 year, Angiitis 30 years TAKE HOME MESSAGES For calcified LM bifurcation lesion, intravascular imaging is strongly recommended to help to determine the PCI strategy. Rotablation is effective to modify the calcified plaque in both MV and SB bifurcation lesion DCB may supply an alternative treatment for bifurcation lesions, thus maybe decrease the stent numbers , thrombosis and restenosis risk of side branch.
European Bifurcation Club 2019, EBC 2019 - Barcelona, Spain AWARD CASE SESSION - VOTING Double Debulking for LM True Bifurcation Lesion Author: Yoshihisa Kinoshita, MD, Toyohashi Heart Center, Japan CONCLUSION Although short DAPT theory becomes widespread recently, we have to keep DAPT after stenting at least 3 to 6 months at least. Especially, it is difficult to stop DAPT within 1 month for the patient who have stent in LM bifurcation lesion. However, there are some patients who cannot continue enough DAPT because of their underlying condition. Aggressive debulking without stenting occasionally brings a favorable result for such patients.
