CASES: TIPS & TRICKS AND COMPLICATIONS

BVS in Bifurcation

Author: Sai Satish, MD, DM, Apollo Hospitals, Chennai, India

PATIENT

  • 44 year old male
  • Recent Onset Angina
  • Negative Troponins
  • Normal LV

ISSUES

  • How easy is it to open the struts of the first BVS??
  • Will I be able to deliver a second bulky Absorb through the struts of the first Absorb??
  • Especially in an active young man with a normal LV, I don’t want to loose the side branch
  • Nor do I want to end up compromising and using a metal DES for the diagonal (loosing the benefit of putting absorbs in the first place……)

FOOD FOR THOUGHT

  1. Is side branch access through BVS safe?
  2. Max balloon size for SB access through BVS??
  3. Ideal positioning of the balloons during the Kiss??
  4. Max balloon pressures during the kiss??
