CONCLUSION

  • Atherosclerotic plaque at bifurcation was predominantly seen in lateral wall (low shear) rather than flow divider (high shear)
  • In bifurcation stenting, lateral wall is likely to have thicker neointima, whereas uncovered struts are significantly more frequent in flow divider than lateral wall regions.
  • Causes of stent thrombosis in bifurcations are: malapposition, uncovered struts, hypersensitivity, protrusion of necrotic core, and strut in ostia of side branch. Among the stent thrombosis cases 6 had CTO.
  • Uncovered struts was the most frequent cause of thrombosis in bifurcation stenting.
  • Stent struts crossing an ostial side branch can lead to thrombotic events. Therefore, KBI (kissing balloon inflation) should be performed in all cases.
  • Stent thrombosis is seen an acute, subacute and late phenomenon after 2 stents strategies
