DEBATE – THE CASE OF DISTAL LEFT MAIN STENOSIS

The case of distal left main stenosis: Should we treat it? – No

Author: W. Wijns, MD, Aalst, Belgium

SUMMARY
  • CABG provides long-term mortality benefit (15-20 years) both in trials and “real life”
  • Recent data with PCI and DES are promising but long term follow-up is lacking (maximum 43 months)
  • Randomized data are being collected (Syntax)
  • CABG should always be offered as the first choice
  • Any other attitude is only possible because we do not (yet) live in a litigation environment
